Job Description

Job Summary:
We are seeking an experienced and detail-oriented Activity Coordinator to join our team. The Activity Coordinator will play a crucial role in planning, organizing, and implementing a variety of engaging activities for our program participants. In addition to the previous responsibilities, the Activity Coordinator will be responsible for conducting activity assessments and documenting the Minimum Data Set (MDS) accurately and efficiently. We are looking for someone who is highly organized, possesses excellent communication skills, and has a strong background in activity coordination and documentation.

Responsibilities:
- Plan, schedule, and coordinate a wide range of activities for program participants, ensuring they are age-appropriate, enjoyable, and meaningful.
- Conduct activity assessments to understand the preferences, interests, and individual needs of each program participant.
- Ensure accurate documentation of the Minimum Data Set (MDS) related to activities, complying with all regulatory requirements.
- Create and distribute comprehensive activity calendars, incorporating the preferences and abilities of program participants.
- Prepare and set up activity areas with the necessary materials, equipment, and supplies.
- Encourage and motivate program participants to actively participate in activities, promoting social interaction, mental stimulation, and physical well-being.
- Assist in the development and implementation of special events and outings, ensuring safety and enjoyment for all participants.
- Maintain accurate records and documentation of attendance, program evaluations, and individual progress.
- Collaborate with team members, including healthcare professionals, to ensure an inclusive and supportive environment for all participants.
- Provide guidance, training, and support to volunteers and staff assisting with activities.
- Stay updated on current trends, research, and best practices in activity coordination, continuously improving the quality and effectiveness of our program offerings.
- Adhere to ethical guidelines and maintain confidentiality of participant information.

Qualifications:
- High school diploma or equivalent required
- Strong organizational and time management skills, with the ability to prioritize multiple tasks efficiently.
- Excellent communication and interpersonal skills, with the ability to relate to people of various backgrounds and ages.
- Demonstrated creativity and ability to develop engaging and innovative activities.
- Ability to work collaboratively as part of a team and adapt to changing needs and schedules.
- Proficiency in Microsoft Office Suite and other relevant software applications.
